-My Heroes Introductory Packet for PT, OT, & SLP

-My Heroes Introductory Packet for Adaptive Riding

-Medical History and Physician Consent Form

-Therapy Prescription Form example

2. Fill out the My Heroes Waiting List Registration Form. You can either upload the required documents to this form or fax them to (888) 551-6210.

3. You will be contacted to schedule your initial visit as soon as we have openings available!
